To help expedite your visit to Eastern Shore Urgent Care, print and fill out forms included in the PDF below. Please submit your forms at check-in. All new patients should also bring a photo ID and your insurance card. If your current address is different than the address on your ID, please bring a utility bill or other piece of mail with your name and current address. If you don't have insurance, there is a $125 deposit due at registration.
